Qualified Testers and Cross-Connection Specialists
The City of Milpitas’ Cross-Connection Control Program maintains a list of Qualified Backflow Prevention Assembly Testers and Qualified Cross-Connection Specialists approved to perform required performance testing and hazard assessments on all backflow prevention assemblies and water services in our City’s service area. The City will only accept backflow test results (Testers) and hazard assessment reports (Specialists) from testers and specialists that are being maintained on our Qualified Lists. Testers and specialists must receive confirmation from the Cross-Connection Specialist of their status on the Qualified List prior to testing an assembly or performing a hazard assessment.
How to Submit Backflow Performance Tests
Test reports are due to the City within 60 days from the City's first notice to the water customer. If an extension is required, the customer is responsible for contacting the Cross-Connection Specialist to explain the reason(s) for an extension, but approval is not guaranteed.
Electronic Test Reports
Required for all routine tests, repairs & re-tests of existing assemblies.
Qualified testers and designated signatories must register and submit test reports online through the City's Backflow Prevention Assembly Test Entry Portal. Note: Written/Scanned Test Reports will NOT be accepted

Backflow Asset Tags - Coming Soon
Backflows Asset Tags issued by the City of Milpitas must be affixed to backflow assemblies and scanned into the online database by a qualified tester. Backflow Asset Tags are available for purchase from the City for the amount listed in the City's Master Fee Schedule. Payments for backflow asset tags are taken at City Hall by the City's Finance Department located at Milpitas City Hall (455 E Calaveras Blvd, Milpitas, CA 95035 1st Floor) and issued by the Cross-Connection Specialist.
Qualified Backflow Prevention Assembly Testers
Qualified Testers must complete and submit a Qualified Backflow Prevention Assembly Tester Application Packet and receive approval prior to performing any tests in Milpitas. Testers and designated signatories whose application is approved and accepted to be on our Qualified List agree to the City’s Qualified Backflow Prevention Assembly Tester Terms & Conditions.
Qualified Cross-Connection Specialists
To be included on Milpitas’ list of Qualified Cross-Connection Specialists, specialists must submit a completed application and agreement, have on file & provide proof of the following items. Expired documentation will not be accepted, and it is the Specialist’s responsibility to provide updated documentation prior to expiration to be maintained on the list.
Application Acceptance Period: CLOSED UNTIL AUGUST 2026
Current Active Period: November 1, 2025, to October 31, 2026, for both Testers and Specialists
