    This form should be filled out by an individual who is directly involved in working with Qualified Testers/ Specialists for annual backflow prevention assembly (BPA) performance testing, repairs and/or replacements, hazard assessments, or has been appointed as a User Supervisor at one or more locations. A separate form for each site with multiple BPA’s is required.
